[Circulation] work authorization forms

Hello Everyone,

I received all of your Work Authorization forms from Student 
Employment today.  I've filled out all the sections that I needed to, 
now you just need to pick them up and sign them.  I have them posted 
in a folder on the circulation bulletin board.  Most of you need to 
fill out tax forms too - these are attached to your employment 
forms.  Please return all forms back to me when you have completed 
them.  I will then  photocopy them and send them to Student 
Employment.  Please remember that you cannot be paid until your 
employment contract is returned to Student Employment.

For returning workers, please make sure to read the New For Fall 04 
memo.  There are copies at each terminal, and there is also one 
posted on the bulletin board.
